The Award

Award recipients will receive the following:

• An official President's Volunteer Service Award pin.
• A personalized certificate of achievement.
• A note of congratulations from the President of the United States.

Award Criteria

• The President's Service Awards are given to members /member group who provided a positive contribution / impact on CHITR's community, including (but not limited to) the online forum, CHIITR Journal, International PACS Administrator Day etc.

• Any member may self-nominate or nominate another member for a President's Service Award.

• To submit a nomination, please contact your community manager for information.

• Previous contribution to ClubPACS and CHITR can be counted towards the number of hours required for the awards. (see the relevant award levels to determine the total number of hours required)

• Contribution out of CHITR but provides a positive impact towards the Health IT industry in general can write in to request for the inclusion of relevant hours but the records must be documented / verifiable.

• Note: There will be some basic administrative and postal charges to process and deliver the package to you.

Presidential Volunteer Service Award Criteria - Bronze Level

Young Adults: 100 to 174 hours
Adults: 100 to 249 hours
& Groups: 200 to 499 hours

Presidential Volunteer Service Award Criteria - Silver Level

Young Adults: 175 to 249 hours
Adults: 250 to 499 hours
 & Groups: 500 to 999 hours

Presidential Volunteer Service Award Criteria - Gold Level
